You can use or adapt this incident / accident / abuse report form for your own project. It can be useful to have a staff member fill this out whenever a child protection incident happens, for example:

• Injury/accident in the skatepark
• A child discloses abuse by family members / educators / other
• A staff member witnesses abuse of a student by another staff member or student
